The Nutrition Care Specialist will be responsible for providing high-quality nutritional care and counseling to patients, residents, and clients at Prairie Mountain Health. This role will involve collaborating with healthcare teams to develop and implement effective nutrition care plans, providing education and support to patients and staff, and contributing to the development of programs and policies related to nutrition care.
Key Responsibilities:
- Conduct comprehensive nutrition assessments to identify patient needs and develop personalized care plans.
- Collaborate with healthcare teams to develop and implement nutrition care plans, ensuring alignment with current best practice evidence and standards of practice.
- Provide education and support to patients, residents, and clients on nutrition-related topics, including healthy eating habits and disease management.
- Develop and maintain accurate and comprehensive patient records, adhering to established standards and protocols.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving nutrition care outcomes.
- Contribute to the development of programs, policies, and procedures related to nutrition care, ensuring alignment with organizational goals and objectives.
- Collaborate with other healthcare professionals to identify areas for quality improvement and develop strategies for enhancing nutrition care services.
Requirements:
- Registered Dietitian (RD) or eligible for registration with the relevant professional organization.
- Strong knowledge of nutrition science, including human nutrition, biochemistry, and physiology.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, staff, and other healthcare professionals.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Strong analytical and problem-solving skills, with the ability to interpret data and develop effective solutions.
