Job Description:

We are seeking highly skilled and compassionate Registered Nurses (RNs) or Registered Practical Nurses (RPNs) to join our dynamic nursing team. As a Community Nurse- Visiting with CarePartners, you will have the opportunity to work in a variety of settings, including in-home, clinic, retirement home, or school, providing exceptional nursing services to patients and their families.

Responsibilities:

  • Develop and maintain effective care plans with respect to community health services
  • Provide nursing services to patients in a variety of settings
  • Communicate and liaise with patients, family members, and other members of the interdisciplinary care team
  • Perform other clinical duties as required

Required Skills and Qualifications:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
