$75,000 - $95,000 per year, based on experience and qualifications.

Job Description

We are seeking a skilled Home Health Nursing Specialist to join our team. As a member of our interdisciplinary care team, you will be responsible for developing and implementing individualized care plans for patients with complex health needs. Your expertise will enable you to provide comprehensive nursing services in various settings, including in-home, clinic, and retirement home environments.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans to meet patients' goals and objectives
  • Provide skilled nursing services, including wound care, medication management, and education
  • Liaise with interdisciplinary care teams to ensure seamless care coordination

Required Skills and Qualifications:

  • Current registration as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO)
  • Minimum 2 years of experience in home healthcare or related field
  • Excellent communication and interpersonal skills
  • Able to work independently and collaboratively as part of a team
