Job Summary:
We are seeking a highly skilled Community Health Nurse Specialist to join our team at Keewaytinook Okimakanak. As a key member of our public health and home care program, you will provide essential nursing services to clients in their homes, while also contributing to the development and implementation of community-based health initiatives.
About the Role:
The successful candidate will work collaboratively with healthcare professionals, community leaders, and other stakeholders to deliver high-quality patient care and support the overall well-being of our clients. This includes assessing client needs, developing personalized care plans, and providing ongoing administrative and clinical supervision to our team of Personal Support Workers (PSWs).
Responsibilities:
- Provide in-home nursing services to clients, including acute care nursing, wound management, medication monitoring, and chronic disease management
- Conduct assessments of client needs and develop goal-based, client-centered care plans
- Deliver training and education to PSWs on various topics, including advanced foot care and mental health coordination
- Collaborate with healthcare professionals and community leaders to develop and implement community-based health initiatives
- Prepare and submit reports on program activities and outcomes
Requirements:
- Bachelor's degree in Nursing or equivalent experience
- Minimum two years of experience providing nursing care in-home or in a community setting and public health
- Advanced Foot Care certification; an asset
- Excellent verbal and written communication skills in English; ability to speak Oji-Cree or Ojibway is an asset
- Basic computer literacy skills and willingness to learn new systems
