Job Summary
We are seeking an experienced Licensed Practical Nurse to join our team as a Nurse Care Coordinator in one of our beautiful long-term care facilities in Burnaby, BC. As a key member of our care team, you will be responsible for delivering high-quality nursing care to residents while ensuring their safety and well-being.

Key Responsibilities:
  • Conduct thorough assessments and develop personalized care plans for residents
  • Administer medications and provide direct supervision of care staff as needed
  • Provide personal care to patients including assistance with bathing, dressing, and toileting needs
  • Collaborate with interdisciplinary teams to ensure seamless care coordination


Requirements:
  • Graduation from an accredited Licensed Practical Nursing Program
  • Current registration with the British Columbia College of Nurses & Midwives (BCCN&M)
  • Current CPR and First Aid certification
  • Excellent communication and interpersonal skills
  • Knowledge of dementia care practices is an asset
