VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. We are seeking a skilled Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our UHN Integrated Care Program.

Key Responsibilities
  • Assess, monitor, and make care planning/coordinating decisions for clients
  • Evaluate care plan interventions through ongoing reassessment and evaluation results
  • Ensure the quality of care for patients by identifying and acting on factors that affect practice autonomy and delivery of quality care
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families
  • Support client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement
  • Maintain documentation standards according to the College of Nurses of Ontario
Requirements
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (RNAO/WeRPN)
  • RN diploma/degree or RPN diploma from a recognized university or college
  • Hold a valid First Aid or CPR Certification
  • A moderate level of wound care or venipuncture knowledge and experience is an asset
  • Acute Care and Medical-Surgical nursing experience is an asset
  • Knowledge of community nursing is an asset
  • Demonstrate effective communication (written and verbal) and documentation skills
  • Demonstrate reliable attendance, punctuality, and availability for the role
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care
  • A valid Ontario driver's license (G License) and a reliable vehicle for traveling is required
