We are seeking a highly skilled and experienced Nurse Specialist in Wound, Ostomy & Continence to join our team at CarePartners. As a NSWOC Nurse Specialist, you will play a critical role in providing wound and ostomy consultation services to patients in our clinics and community.
Key Responsibilities- Provide wound and ostomy consultation services to patients in our clinics and community
- Use your expertise in wound and ostomy care to promote self-management by patients and lead the interdisciplinary team to achieve optimal patient outcomes
- Develop and implement individualized care plans to address the unique needs of each patient
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
- Stay up-to-date with the latest research and evidence-based practices in wound and ostomy care
- Registered Nurse (RN) diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
- Several years of escalating experience managing wounds and ostomies
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver's license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- To protect our patients, we require a current vulnerable sector check and a clear background check
