Job Title: Community Mental Health Worker 2

About the Role:

We are seeking a skilled Community Mental Health Worker 2 to join our Youth Intensive Case Management team at the Outreach Community Health & Services Centre in Kelowna, B.C. As a member of our interdisciplinary team, you will provide case management services to high-risk youth with problematic substance use, concurrent disorders, and/or mental illness.

Key Responsibilities:

Provide case management services to high-risk youth
Work collaboratively with the interdisciplinary team to develop and implement treatment plans
Conduct assessments and provide counseling services to youth and their families
