Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our Community Wellness team at Mount Carmel Clinic. As a key member of our interdisciplinary team, you will provide a range of client care services to ensure client engagement and reduce or remove barriers to care.
Key Responsibilities- Provide extended community outreach and health education to clients
- Develop and implement case management plans to support client needs
- Offer treatment support, counseling, and group facilitation to promote wellness and disease prevention
- Collaborate with community resources to provide advocacy and support services
- Work with clients to promote harm reduction and trauma-informed care
- Bachelor's Degree in Social Work or related field
- Registration with the Manitoba College of Social Workers
- Minimum 5 years of experience working as a Social Worker
- Experience working in addictions, mental health, crisis intervention, and cognitive behavioral therapy
- Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
- Experience working in an inner-city community setting
- Knowledge and experience working with Indigenous communities and protocols
- Ability to work with students and/or volunteers
- Links/relationships with community resources
