About the Role

Vancouver Coastal Health is seeking a skilled and compassionate Caregiver Assistant to join our team. As a key member of our community health team, you will provide home support services to clients, working under the direction of the Supervisor/Coordinator/Manager or delegate.

Key Responsibilities

  • Provide home support services to clients, including assistance with daily living activities, observation, and reporting of significant changes in client presentation.
  • Collaborate with the healthcare team, clients, and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Motivate, encourage, and involve clients in their care, providing input and feedback in the care planning process.
  • Work with clients to set and achieve therapeutic goals, promoting independence and self-care.
  • Develop and maintain positive relationships with clients, families, and healthcare teams, providing culturally sensitive and competent care.

Qualifications

  • Graduation from a recognized Health Care Assistant Program.
  • Current/Active Registration with BC Community Health Worker and Care Aide Registry.
  • Experience interacting with persons with dementia, geriatric, and/or palliative populations will be an asset.
  • Valid BC Driver's license and access to a vehicle, as local area travel may be required.
