Wound Care Specialist Nurse

About This Role:
We are seeking a skilled Skin Wellness Associate Nurse to join our team at SE Health. As a SWAN Nurse, you will work in the Kawartha Lakes and Haliburton area, providing expert care to clients requiring wound, ostomy, and continence services. Your responsibilities will include developing treatment recommendations, collaborating with interprofessional teams, and evaluating plans of care to ensure optimal client outcomes.
Key Responsibilities:
- Formulate treatment recommendations in collaboration with the interprofessional team.
- Develop a plan of care based on the holistic assessment of clients who require wound, ostomy, and/or continence care.
- Evaluate the plan of care and escalate to the Nurse Specialized in Wound, Ostomy, and Continence (NSWOC) when clinically indicated.
- Involve the K2P Wound Care Resource Nurses (K2P WCRN) in the evaluation process as appropriate.
- Provide education and mentorship experiences to build wound, ostomy, and continence knowledge and capacity within SE Health.
- Provide consultation to care providers within the organization.
- Coordinate and collaborate with interprofessional teams across the continuum of care.
- Support quality healthcare through teaching, mentoring, and quality monitoring.
- Optimize client outcomes through review of quality improvement (QI) reports to prioritize needs.
- Identify nursing trends and issues related to wound, ostomy, and continence management.
- Assist in the development of information tools and educational initiatives for care providers.
- Collaborate with professional nursing associations to develop and enhance the delivery of healthcare for Canadians.
- Participate in local funder initiatives/working groups as required.
Requirements:
- A willingness to participate in further education programs/courses to remain current.
- Adoption of the SE model for wound care delivery through mentorship of K2P WCRN.
- A demonstrated appreciation for quality improvement processes.
- A valid driver's license and vehicle (travel required).
- A willingness to attain required SE Health certifications.
- Availability to work days, evenings, nights, and every other weekend.
- Strong computer skills in MS Word, MS Excel, Email, etc.
- An ability to use computer word processing, power point, email, and source information on the internet is preferred.
