Job Summary

We are seeking a motivated LPN to work independently and collaboratively as part of a multi-disciplinary team. Under the direction of the Home & Community Care Manager, you will screen, plan, and coordinate care for clients in community settings, ensuring quality of care by supporting and directing the practice of community health workers and residential care aides.

Key Responsibilities
  • Screen, plan, and coordinate care for clients in community settings
  • Ensure quality of care by supporting and directing the practice of community health workers and residential care aides
  • Work collaboratively as part of a multi-disciplinary team
Requirements
  • College of Licensed Practical Nurses approved pharmacology course
  • Head-to-toe assessment
  • Minimum two years' acute care experience or an equivalent combination of training and experience
  • Class V BC Diver's License
  • Certificates in CPR and First Aid
  • Ability to communicate effectively both verbally and in writing
