Job Title: Rehabilitation Service Specialist

Lifemark is seeking a highly skilled Rehabilitation Service Specialist to join our team. As a key member of our rehabilitation services team, you will play a critical role in delivering high-quality rehabilitation services to veterans of the Canadian Armed Forces and their families.

Key Responsibilities:
  • Develop and implement comprehensive rehabilitation plans to support veterans in achieving their optimal wellness and independence.
  • Conduct assessments and make recommendations to support a participant's rehabilitation needs.
  • Collaborate with medical, psycho-social, and vocational rehabilitation professionals to ensure a holistic approach to rehabilitation.
  • Empower participants to problem-solve and explore options for community-based interventions and resources.
  • Identify barriers or risk factors contributing to suboptimal outcomes and develop strategies to address them.
Requirements:
  • Bachelor's degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field.
  • Five years of experience in case management or a health professional designation (Registered Social Worker, Kinesiologist, Occupational Therapist, Physical Therapist, Registered Nurse).
  • Minimum one year of experience in the direct provision of medical, psycho-social, or vocational rehabilitation services.
  • Experience working with individuals with mental or physical health problems creating barriers in life.
  • Employment conditional on obtaining federal security clearance - Reliability Status.
  • Reliable vehicle and ability to travel.
Preferred Criteria:
  • Bilingual (French/English).
  • Experience working with employers, community resources, and training institutions.
  • Preference given to qualified former CAF members.
