Nurse Practitioner

We are seeking an experienced and dedicated Registered Nurse to join our team as a Nurse Practitioner. In this role, you will work closely with patients, families, and healthcare professionals to provide high-quality patient care.
About the Role- As a Nurse Practitioner, you will be responsible for assessing patients' health needs, developing and implementing individualized care plans, and providing ongoing monitoring and evaluation of patient outcomes.
- You will also be expected to manage home care cases, deliver health education programs, and provide consultative services to patients and their families.
- Your excellent communication and interpersonal skills will enable you to work effectively with patients, families, and healthcare teams to achieve optimal health outcomes.
- Assess patients' physical and emotional health needs
- Develop and implement individualized care plans
- Provide ongoing monitoring and evaluation of patient outcomes
- Manage home care cases
- Deliver health education programs
- Provide consultative services to patients and their families
- Supervise licensed practical nurses and other nursing staff
- Degree in Nursing (Bachelor's or higher)
- Licensure as a Registered Nurse by provincial or territorial authorities
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Manual dexterity and ability to stand for extended periods

- Hours: 30 to 40 hours per week
