About the Role

VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. As a Registered Nurse (RN) or Registered Practical Nurse (RPN) on our UHN Integrated Care Program, you will have the opportunity to make a meaningful difference in the lives of our clients.

Key Responsibilities
  • Assess and monitor clients' needs, developing personalized care plans to ensure their safety and well-being.
  • Deliver high-quality, client-centered care in the comfort of clients' homes, including wound care, IV therapy, and administering injections.
  • Collaborate with our multidisciplinary team to provide comprehensive care and support to clients and their families.
  • Document client interactions and care plans accurately and efficiently, adhering to our standards and guidelines.
Requirements
  • Current registration with the College of Nurses of Ontario (CNO).
  • Membership with the Registered Nurses' Association of Ontario (RNAO) or the Registered Practical Nurses Association of Ontario (WeRPN) is preferred.
  • RN diploma/degree or RPN diploma from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • Experience in acute care, medical-surgical nursing, or community nursing is an asset.
