Vancouver Coastal Health is seeking a skilled Social Worker to join our Home Health team in North Vancouver, BC. As a Case Manager, you will provide clinical and consultative social work services to clients and families experiencing complex health care needs.
Key Responsibilities- Conduct functional and psychosocial assessments to identify client needs and develop care plans.
- Provide individual, family, and group counseling services to promote client well-being and independence.
- Maintain accurate client records and participate in interdisciplinary clinical conferences.
- Collaborate with healthcare professionals and community resources to ensure comprehensive care and support.
- Develop and implement project action plans to achieve client-centered outcomes.
- Master's Degree in Social Work.
- Three years' recent experience in case management, preferably with geriatric clients and adults with chronic and acute illness.
- Current full registration with the British Columbia College of Social Workers.
- Valid BC Driver's License.
