We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Mental Health and Addiction Specialist. As a key member of our Home and Community Care Support Services team, you will play a vital role in providing essential health-related advice and support to educators within schools and developing individual care plans for clients with mental health and addictions needs.
Key Responsibilities:- Provide mental health and addiction services to children and youth in schools and community settings
- Develop and implement individualized care plans in collaboration with mental health workers and other healthcare professionals
- Conduct assessments and provide treatment recommendations for clients with mental health and addiction issues
- Collaborate with educators, parents, and other stakeholders to provide support and resources for clients
- Participate in multidisciplinary teams to develop and implement care plans and services
- Current registration with the College of Nurses of Ontario (CNO)
- Minimum of two (2) years of experience as a Registered Nurse in a mental health or addiction setting
- Knowledge of mental health and addiction services and resources in Ontario
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in a Windows environment
- Experience working with diverse patient populations, including multicultural, homeless, palliative, and acquired brain injury (ABI) clients
- Case management experience or recent related community experience
- Ability to speak French or another second language
