Job Summary:

Southbridge Care Homes is seeking a compassionate and dedicated Personal Support Worker to join our team. As a key member of our care team, you will play a vital role in providing high-quality care and support to our residents.

Key Responsibilities:

  • Provide direct care and support to residents, including assistance with daily living activities, personal care, and mobility.
  • Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs and preferences of each resident.
  • Provide emotional and social support to residents and their families, including palliative care and end-of-life support.
  • Participate in ongoing education and training to enhance your skills and knowledge in care planning and delivery.
  • Contribute to a positive and supportive team environment that promotes resident-centered care and well-being.

Requirements:

  • PSW Program Certificate or Diploma that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to resident-centered care and a patient-focused approach.
  • Knowledge of care planning systems and documentation, with the ability to maintain accurate and up-to-date records.
