Patient-Centered Care: At North East Local Health Integration Network, we believe that patient-centered care is essential to delivering high-quality healthcare services. As a Care Coordinator, you will work collaboratively with patients and their families to assess care needs, determine eligibility for services, and develop individual care plans. You will also link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.

About the Role: This is a temporary, full-time opportunity available immediately to October 31, 2025. The successful candidate will be based in Timmins and will work closely with our team to deliver patient-centered care. You will have the opportunity to work with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.

Key Responsibilities:
  1. Assess patient care needs and determine eligibility for services
  2. Provide access to community resources
  3. Engage with patients, caregivers, and healthcare practitioners to deliver patient-centered care
  4. Develop individual care plans
  5. Link patients with service providers
  6. Coordinate and monitor care plan delivery
  7. Establish a helping relationship with patients and their families


Requirements: To be successful in this role, you must have membership in good standing with the applicable regulatory body, recent experience in community health or a related field, knowledge of the healthcare delivery system and community resources, excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, effective time management, prioritization, and organizational skills, and the ability to work independently and co-operatively in a busy multidisciplinary environment.
