Home Care Support Specialist

Job Description:

This role requires a detail-oriented and compassionate individual to provide home care support services to clients in need. The Home Care Support Specialist will facilitate client transportation, assist with hospital discharge instructions, and complete errands to meet client physical needs.

Key Responsibilities:
  • Transport clients to their homes using local transportation services or agency vehicles when necessary.
  • Support clients in understanding and implementing hospital discharge instructions, creating personalized care plans as needed.
  • Complete errands related to client physical needs, such as nutrition, medication, medical supplies, and equipment, in accordance with established service plans.
  • Plan and arrange for home management duties like cooking, cleaning, laundry, etc., in advance of discharge when identified.
  • Facilitate client and caregiver engagement by providing relevant information on transition-related issues from hospital to home.
  • Communicate recommendations and supports clients, caregivers, and those in the circle of care to address safety concerns.
  • Provide companionship to seniors suffering from social isolation.
  • Conduct risk assessments of clients' homes and evaluate their risk of falls using standardized tools.
  • Support post-discharge clients to remove barriers and reduce risks in accessing resources and follow-up appointments.
Requirements:
  • A minimum of two years of post-secondary education in social services, gerontology, therapy assistance, or a related field.
  • At least three years of job-related experience or an equivalent combination of education and experience.
  • Previous knowledge of medical and technical terminology is an asset; successful candidates without prior knowledge are expected to develop it in the role.
  • Fluency in English and French is required.
  • Excellent computer skills, including familiarity with Microsoft Office.
  • Knowledge of CIMS and other databases is considered an asset.
  • A valid driver's license and access to a reliable vehicle.
Working Conditions:

The work involves a combination of office, hospital, community, and client home settings, requiring some physical activity, bending, stretching, and repetitive motions.

This role may involve wheelchair pushing, helping clients get in and out of vehicles, and other physically demanding tasks.

Candidates must be eligible to work in Canada and pass a pre-employment screening process, including an Enhanced Police Information Check (E-PIC).
