About the Role

The Long-term Care Coordinator is a key member of our team, responsible for coordinating day-to-day operations and ensuring high-quality individual care within our designated clinical areas. This role supports the seamless movement of individuals through the healthcare system, facilitating access to appropriate resources and services.

Key Responsibilities
  • Supervise and mentor staff to ensure clinical excellence and adherence to policies and procedures
  • Develop and implement administrative systems, including policies, procedures, and standards of individual care
  • Liaise with the interdisciplinary team to identify, coordinate, and utilize resources efficiently
  • Plan and deliver staff educational programs to enhance clinical decision-making and quality of care
