Job Summary:

Schlegel Villages is seeking a skilled Registered Practical Nurse to join our team as a Village Care Specialist. As a key member of our multidisciplinary team, you will provide high-quality care to our residents, working collaboratively with our healthcare professionals to deliver holistic care and support.

Key Responsibilities:

  • Provide direct care to residents, including administering medications, wound care, and other treatments as required.
  • Develop and implement individualized care plans in collaboration with residents, families, and healthcare professionals.
  • Conduct regular assessments of residents' physical, emotional, and social needs, and make recommendations for care and support.
  • Collaborate with our healthcare team to ensure seamless care transitions and effective communication.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
  • Current Heart Save Level of CPR certification.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality care and support.

Preferred Qualifications:

  • MDS credentials.
  • Knowledge of relevant policies, procedures, legislation, regulations, and best practices applicable to the retirement and long-term care sector.
