Care Manager Job Summary

At Park Place Seniors Living, Inc., we are seeking a highly skilled and compassionate Care Manager to join our team. As a key member of our nursing care team, you will be responsible for recruiting, leading, and supervising the care team to ensure the delivery of high-quality, resident-centered care.

Key Responsibilities:
  1. Recruit, orient, and lead the nursing care team to ensure effective care planning and delivery.
  2. Provide leadership and guidance to the care team, promoting a collaborative and respectful approach to care.
  3. Ensure adequate staffing levels and monitor team collaboration to ensure quality care.
  4. Lead quality improvement initiatives and participate in site committees to focus on quality of care and safety assurance.
  5. Ensure accurate documentation of assessments and care provision according to professional standards.
  6. Lead the nursing care team in planning, organizing, and establishing priorities.
  7. Comply with Occupational Health and Safety policies and safe work practices.
  8. Educate employees on workplace violence and harassment policies.
Requirements:
  • Fluent communication in English, both verbally and in writing.
  • Establish professional relationships with residents, families, and the care team.
  • Effectively manage purposeful change consistent with Park Place's vision.
  • Apply continuous quality improvement tools in a dynamic environment.
  • Demonstrate computer skills and knowledge of nursing-related care equipment.
