About the Role

CarePartners is seeking a skilled Community Support Worker to join our team in the High Intensity Supports at Home (HISH) program. As a CSW, you will work with patients and their families to provide wrap-around care and support while they transition through the healthcare system.

Key Responsibilities

  • Assist with daily living activities, such as personal care, household tasks, and medication assistance
  • Provide cognitive, social, and emotional support to patients
  • Prepare nutritious meals and snacks, using proper food preparation and hygiene practices

Requirements

  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
  • Current vulnerable sector check screening and a clear background check
  • Availability to work non-traditional hours, including early morning, evenings, and alternate weekends
