Key Responsibilities- Assess and Plan Care: Conduct thorough assessments of patients' physical and emotional needs, develop and implement individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care delivery.
- Provide Direct Care: Deliver hands-on nursing care, including administering medications, wound care, and vital sign monitoring, to ensure the physical and emotional well-being of our residents.
- Lead and Mentor: Supervise and mentor Licensed Practical Nurses and other nursing staff, providing guidance and support to ensure the highest standard of care.
- Collaborate with Interdisciplinary Team: Work closely with physicians, therapists, and other healthcare professionals to coordinate patient care and ensure comprehensive support.
- Document and Communicate: Accurately document patient information, communicate effectively with residents, families, and healthcare team members, and maintain confidentiality at all times.
- Education and Experience: Current registration with the College of Nurses of Ontario, 3 years of experience in long-term care, and a strong background in gerontology.
- Skills and Qualities: Excellent communication and interpersonal skills, ability to work in a fast-paced environment, and a passion for delivering high-quality patient care.
