Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Northwood in Bedford.
About the RoleThis is a dynamic opportunity for a dedicated professional to provide high-quality care to our residents. As a Care Coordinator, you will be responsible for delivering person-centered care, coordinating care plans, and collaborating with interprofessional teams.
You will have the opportunity to work in a fast-paced environment, where no two days are the same. Your strong clinical skills, excellent communication abilities, and problem-solving aptitude will enable you to make a meaningful impact on the lives of our residents.
Responsibilities- Daily Operations: You will be responsible for conducting comprehensive assessments of clients, maintaining accurate and timely documentation, and addressing mental health and social issues.
- Team Collaboration: You will work closely with interprofessional teams to coordinate care plans, ensure effective communication, and promote a safe environment for clients and staff.
- Clinical Skills: You will maintain up-to-date clinical skills, assess patients' medical information, and record vital signs.
To be successful in this role, you will need to have a Registered Nurse (RN) license from the Province of Nova Scotia, a minimum of 6 months of related experience, and strong clinical, communication, organizational, and interpersonal skills.
We are looking for a dedicated professional who can work independently, demonstrate initiative, and attend work on a regular and consistent basis.

What We OfferWe offer a competitive salary of $85,000 - $110,000 per year, depending on experience, and a range of benefits including health and dental insurance, paid time off, and opportunities for professional growth and development.
