Care Coordinator for Complex Care and Recovery

We are seeking a highly skilled Registered Nurse to join our team as a Care Coordinator for the Complex Care and Recovery Program. This is a part-time, temporary (12 months) position located at the Queen Street Site.
About the RoleThe successful candidate will provide client-centered psychiatric and general nursing care in the Structured Observation and Treatment Unit and Women's Secure Forensic Unit. As a member of an interprofessional team, you will work collaboratively with clients, families, and healthcare professionals to develop, implement, and evaluate interprofessional plans of care.
Responsibilities- Provide high-quality nursing care to clients with complex mental health needs, including those involved in the criminal justice system.
- Perform client assessments, develop, implement, and evaluate interprofessional plans of care.
- Liaise with internal and external services and community agencies to coordinate care for clients.
- Participate in clinical supervision, education, and research activities.
- Document client information and communicate effectively with other team members.
- Member in good standing with the College of Nurses of Ontario.
- B.C.L.S. (CPR) certificate.
- Experience working with forensic patients and knowledge of risk assessment tools (an asset).
- Excellent communication and interpersonal skills.
- Effective leadership, decision-making, organizational, and problem-solving skills.
This part-time, temporary position offers a competitive salary of $ per year. The salary is commensurate with experience and qualifications. Additional hours may be available based on operational needs.
This position is part of the ONA Bargaining Unit and involves shift rotation and weekend work. We offer a healthy work environment that values diversity, teamwork, and collaboration.
