About the Role: We are seeking a dedicated and compassionate Support Worker to join our team at Prokare Support Services Inc.

Job Summary: The successful candidate will be responsible for providing high-quality support services to clients, ensuring their safety and well-being. This role involves preparing reports, appraising clients' needs, and implementing life skills workshops.

Responsibilities:

  • Prepare comprehensive reports on client progress and needs
  • Assess and identify clients' eligibility for specific services
  • Design and implement life skills workshops tailored to clients' requirements
  • Conduct regular follow-up assessments to monitor client progress
  • Perform household chores and prepare meals as needed
  • Resolve conflict situations in a professional and empathetic manner
  • Supervise children's visits with parents to ensure their safety and security
  • Develop and implement substance abuse treatment programs
  • Implement behavior management programs to support clients' development
  • Lead individual and group counseling sessions to promote emotional well-being
  • Maintain accurate records of clients' special needs and services provided
  • Liaise with other social services agencies and healthcare providers to coordinate care
  • Collaborate in developing service intervention logic models and outcome measures
  • Analyze program statistics to inform evaluation and research initiatives
  • Support clients with special needs in daily activities
  • Accompany clients to appointments and social outings as required

Requirements:

  • 1-2 years of experience in a related field, preferably in social services or healthcare
  • Possession of a college diploma or equivalent in a relevant field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to adapt to changing circumstances and priorities
  • Basic computer skills and proficiency in Microsoft Office
  • Bilingualism in English and French is an asset

Working Conditions:

This is a permanent, full-time position working 30-44 hours per week. A competitive salary range is between $40,000 and $60,000 annually, commensurate with experience.
