**Job Summary:**

We are seeking a compassionate and caring pharmacist to join our interprofessional team at Scarborough Centre for Healthy Communities (SCHC). As a Community Health Pharmacist, you will play a vital role in supporting appropriate medication use for registered clients and promoting client safety through audits.

**The Role:

This part-time, permanent position requires a pharmacist with clinical experience within a community-based healthcare setting. You will work closely with an interdisciplinary team to complete medication reconciliations, review prescribed medications, provide drug information, and consult with the multidisciplinary team as necessary.

 

**Responsibilities:

* Accurately input and update clients' medication information in electronic health records
* Assist in creating and updating facility medication policies and procedures
* Complete post-discharge medication reconciliation for clients
* Adjust client medications in consultation with the primary care provider or as per medical directives
* Assist in deprescribing to reduce polypharmacy and the use of potentially inappropriate medications (PIMs)
* Create and update medical directives for flu vaccinations and COVID-19
* Coordinate stocking of vaccines, point-of-care testing (POCT), and vaccine refrigeration monitoring
* Participate in workshops to enhance skills and knowledge
* Identify community needs and assist in program development
* Engage in outreach and community work through planning, implementation, and evaluation

 

**Requirements:

Our ideal candidate has:
* A degree in pharmacy from a recognized institution
* Registration with the College of Pharmacists of Ontario
* Clinical experience within a community-based healthcare setting
* Strong communication and interpersonal skills
* Ability to work effectively in a team environment
* Knowledge of legislation and pharmacy-related regulatory requirements for community pharmacy practice
* Familiarity with electronic health record documentation environments
* Willingness to preceptor students from accredited academic facilities
