Job Summary:The Care Coordinator will play a vital role in assessing patient care needs, determining eligibility for services, and developing individual care plans. This is a dynamic and rewarding position that requires strong communication and interpersonal skills, as well as the ability to work collaboratively with patients, caregivers, and healthcare professionals.
Key Responsibilities:- Assess patient care needs and determine eligibility for services
- Develop and implement individual care plans
- Link patients with service providers
- Coordinate and monitor care plan delivery
- Establish a helping relationship with patients and their families
- Membership in good standing with the applicable regulatory body
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
