We are seeking a highly skilled and dedicated Occupational Therapist to join our team at Community Advantage Rehabilitation, a division of pt Health. As a Community Occupational Therapist, you will play a vital role in providing high-quality, community-based rehabilitation services to adults and children in Durham Region.
Key Responsibilities- Provide ministry-funded, community-based Occupational Therapy services to clients in their place of residence.
- Conduct occupational therapy assessments, including interviews, standardized, and informal assessment techniques, to identify client needs and develop personalized care plans.
- Develop and implement evidence-based occupational therapy interventions in collaboration with clients and caregivers.
- Provide education and support to clients and caregivers, linking them to community agencies and resources as needed.
- Complete funding applications and maintain accurate documentation of assessment findings, care plans, progress, and discharge summaries.
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario.
- Experience working in a community setting is preferred, but new graduates are also encouraged to apply.
- Ability to work independently and as part of an inter-professional team.
- Strong communication, interpersonal, and organizational skills.
- Reliable transportation is required for this position.
