Chronic Disease Management Specialist

At Prairie Mountain Health, we are seeking a highly skilled Chronic Disease Management Specialist to join our team.
About the RoleThis is a challenging and rewarding role that requires a strong commitment to providing high-quality patient care. As a Chronic Disease Management Specialist, you will play a critical part in our interdisciplinary primary health care team, working closely with clients, families, and healthcare providers to support the management of chronic diseases.
Key Responsibilities- You will assess the needs of clients, develop and implement individualized care plans, and provide education and support to promote healthy behaviors and self-management of chronic disease.
- You will apply the nursing process, including assessment, planning, implementation, and evaluation, to deliver evidence-based care that meets the unique needs of each client.
- You will work collaboratively with other healthcare professionals, community agencies, and organizations to facilitate referrals, coordinate services, and support the adoption of clinical practice guidelines for chronic disease management.
- You will participate in ongoing professional development, staying up-to-date on best practices and research findings to enhance your skills and knowledge as a Chronic Disease Management Specialist.
To be successful in this role, you must have:
- A bachelor's degree in nursing or a related field.
- Current registration with the College of Registered Nurses of Manitoba.
- Experience working with diverse populations, including individuals with chronic diseases.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
- The ability to prioritize workload, manage competing demands, and maintain confidentiality.
We offer a competitive salary range of $94,000 - $123,000 per year, depending on experience, plus comprehensive benefits, including extended health and dental coverage, group life insurance, and paid time off.
