**About the Opportunity**:
**Please note: This position requires the successful incumbent to strictly adhere to Integrated Chronic Care Services (ICCS) scent-free free policy.**

The ICCS provides comprehensive assessments and care planning for individuals with complex chronic conditions and multi-morbidities such as: environmental illness, chronic fatigue syndrome, fibromyalgia, functional neurologic and gastrointestinal syndromes.

The ICCS model is based on the premise that chronic disease outcomes are better managed by addressing the needs of the whole person with a focus on self-management support, with a particular focus on behaviour change. With a team of interdisciplinary health care professionals, we integrate the physical, psychosocial and environmental needs of individuals by focusing on the person instead of the disease(s)/condition(s), improving functional health and quality of life. Our care team includes physicians, occupational therapists, a nurse practitioner, registered nurse, licensed practical nurse, psychotherapist, and a dietitian with support from an admin team.

Reporting to the Health Services Manager, Integrated Chronic Care Service, the Post COVID Navigator, through an intake/screening process, assesses clients/family needs, develops recommendations, and evaluates the most appropriate services that will assist in meeting the individual/family's goal with persistent Post COVID Syndrome to obtain optimal functional health. Works collaboratively with the client’s family physicians, diverse Nova Scotia Health care team and community supports/agencies across the post COVID care continuum. Supports access to the services and programs that enables successful plans for the client/family. The goal is to enable the client to live in their own community and have issues addressed before they encounter a crisis point and require a visit to emergency and/or hospital admission. The navigator acts as a resource for obtaining answers to questions and concerns and supports client navigation along the evolving Post COVID Care Pathways.

**About You**:
We would love to hear from you if you have the following:

- Bachelor's degree required from an accredited institution in one of the following health care disciplines: Nursing, Occupational Therapy, Physiotherapy.
- Registration with respective professional regulatory association required.
- 3 years’ experience working with care transitions and accessing diverse resources to support client population needs required.
- In-depth knowledge and experience with cross collaboration from acute to community care settings.
- In-depth knowledge of Nova Scotia Health Authority services/programs, in particular services that support functional health and behavior change.
- Excellent verbal and written communication skills.
- Good working knowledge of delivering virtual care.
- Use evidence-based principles of behavior change/motivational/active listening interventions to determine readiness for change and the level of support required.
- Ability to develop and maintain community networks.
- Ability to use data in the evaluation and improvement of the Post COVID Navigator.
- Able to be innovative and creative in order to develop strategies that successfully meet that needs of diverse client, family, and provider populations.
- Competency in computer-based programs (Microsoft Office Programs).
- Acquire knowledge of Nova Scotia Health Authority processes and systems.
- Develop significant relationships/partnerships with Family physician community Post COVID Model Advisory and relevant working groups, Nova Scotia Health Authority services/programs, community agencies, organizations, and groups.
- Flexible and excited to take on a new challenge.
- Valid Nova Scotia Driver's license and access to a reliable vehicle, as some travel is a requirement of the position.
