Trillium Health Partners is seeking a skilled Registered Practical Nurse to join our Cardiac Care team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients with complex cardiac conditions.
Key Responsibilities
- Provide direct patient care to patients with cardiac conditions, including medication administration, wound management, and IV therapy.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop strategies for care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
- Diploma in Practical Nursing from a recognized college or school of nursing.
- Current registration with the College of Nurses of Ontario (CNO).
- Minimum 1 year of acute care experience, preferably in cardiology.
- Telemetry experience an asset.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
