Prairie Mountain Health is seeking a dedicated professional to join our team as a Registered Dietitian. With a strong focus on cardiovascular health and diabetes prevention, this role will play a vital part in implementing the Manitoba Diabetes Strategy across various regions.

The ideal candidate will possess a minimum of three years of clinical experience working with healthcare teams and have certifications such as C.D.E. or similar. A Province of Manitoba Class 5 Driver's License and proficiency in MS Office, Outlook, and internet applications are also required.

This role involves collaborating with partners to develop and implement health promotion strategies, conducting community assessments, and contributing to facilitation, communication, and resource skills for risk reduction and disease prevention.

Key Responsibilities:
- Collaborate with partners to implement the Manitoba Diabetes Strategy and promote cardiovascular health.
- Assess/identify priority needs related to health promotion and prevention of diabetes and cardiovascular disease in the region.
- Lead planning, implementation, and evaluation of health promotion strategies related to diabetes and cardiovascular disease.
- Contribute facilitation, communication, and resource skills to strengthen existing resources for prevention of diabetes and cardiovascular disease.

Educational Components:
- Collaborate with partners to offer education for individuals, families, groups, communities, and healthcare providers regarding disease prevention, management of diabetes and cardiovascular disease, and avoidance of complications.
- Assume a lead teaching/facilitation role through individual assessment, planning, implementation, and evaluation of the nutrition care plan with clients and their significant others.
- Utilize current resources to provide accurate information and maximize client interest, participation, and learning.

Care and Support Services:
- Assume a lead role on an interdisciplinary diabetes and cardiovascular health care team to address education, risk reduction, disease management, and prevention of complications.
- Incorporate best practices information, Community Health Needs Assessment data, and accreditation requirements in planning, implementation, and evaluation.
- Monitor and evaluate process and outcome indicators.
- Identify the required minimum data set, collect data consistently, evaluate the data, and how the data is used.

Effective Communication and Collaboration:
- Apply communication theory and skills effectively.
- Nurture partnerships by sharing and using information, and maintaining effective working relationships with clients, colleagues, supervisor, and partners.

Administrative Functions:
- Report programmatically to Primary Health Care.
- Submit calendars, monthly reports, and statistics.
- Follow policy and collective agreement regarding hours of work and overtime.
