Palliative Care Nurse

Spectrum Health Care is seeking a skilled Palliative Care Nurse to join our team. As a key member of our interdisciplinary care team, you will provide holistic, patient-centered care to patients and their families, focusing on symptom management, coordination of care, and emotional support.
Key Responsibilities:- Work under the supervision of the Neighbourhood Home Care Specialist to provide high-quality care to patients and their families.
- Collaborate with the interdisciplinary team to create personalized care plans based on patient needs and goals.
- Participate in clinical huddles and case conferences to ensure seamless care coordination.
- Practice in accordance with the College of Nurses of Ontario, the Regulated Health Professions Act, and Spectrum Health Care standards, policies, and procedures.
- Travel within assigned regions to provide care to patients in their residences or facilities.
- Current registration and good standing with the College of Nurses of Ontario.
- Minimum of two years of nursing experience.
- Experience working with palliative care patients.
- Successful completion of Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing, or equivalent.
- Current police check suitable for working with vulnerable sectors.
- Valid Ontario driver's license and reliable vehicle with willingness to travel.
- Demonstrated problem-solving and critical thinking skills.
- Proficiency in English, both written and verbal.
- Ability to work independently and prioritize workload, with strong time management skills.
- Physical ability to perform daily duties.
- Proficiency in technology and software, including Word and Excel.
