Patient Care Specialist

RhynoCare is seeking a dedicated and compassionate Patient Care Specialist to join our team. As a Patient Care Specialist, you will play a vital role in providing high-quality care to our residents, ensuring their safety and well-being.
Key Responsibilities- Observation and Support: Observe and support residents who require close supervision due to behavioral or safety concerns.
- Documentation and Communication: Accurately document observations and communicate with the healthcare team to facilitate care planning.
- Assistance with Daily Living: Provide assistance with activities of daily living, such as bathing, dressing, and grooming, under the supervision of registered healthcare professionals.
- Engagement and Activities: Engage residents in meaningful activities, such as reading, storytelling, and other cognitive stimulation exercises.
- Safety and Security: Ensure the safety and security of residents, including those with elopement risks or suicidal tendencies.
- Collaboration and Teamwork: Work collaboratively with the healthcare team to provide comprehensive care and support to residents.
- Education: Minimum Grade 11 or equivalent education as recognized by the Ministry of Education, Province of Ontario.
- Experience: One year of recent related experience in patient care, hospital, long-term care, or complex continuing care setting required.
- Language: Must be fluent in speaking and documenting in English.
- Certifications: Current Non-Violent Crisis Intervention Certification and proof of current immunization for measles, mumps, rubella, varicella (MMRV), diphtheria, pertussis, and hepatitis B.
- Police Check: Willing to undergo Vulnerable Sector Police Check as a condition of hire.
