Responsibilities:- Conduct regular wellness visits for all residents to assess their health and well-being.
- Monitor and follow up with residents who have experienced any falls, accidents, or incidents, documenting and reporting as required.
- Assist in maintaining accurate and up-to-date resident wellness files, ensuring compliance with Sunrise policies and federal and state regulations.
- Collaborate with the Resident Care Director (RCD) and community team to promote infection control standards, including resident testing, training, and skills checks.
- Provide clinical support and assistance to community team members as needed, ensuring seamless care delivery.
- Stay up-to-date with the latest medical supplies and emergency kits, maintaining a safe and healthy environment for residents.
- Participate in Sunrise Quality Assurance Meetings and Clinical Meetings to drive quality improvement initiatives.
- Graduate of an approved college or school of nursing.
- Current BCCNM registration and a valid state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.
- Excellent communication and interpersonal skills, with the ability to teach and guide team members.
- Competent in organizational, time management, and problem-solving skills.
- Proficient in computer skills, Microsoft Office, and Sunrise applications.
