As a key member of our interdisciplinary team, you will be responsible for providing holistic care to patients, incorporating the nursing profession's values of knowledge, theory, and practice.
Key Responsibilities- Assess, plan, implement, and evaluate patient care needs in collaboration with the interdisciplinary team.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure comprehensive care.
- Collaborate with various health disciplines and family members to achieve patient goals.
- Provide education and support to patients and families to promote health and well-being.
- Participate in quality improvement initiatives to enhance patient care and services.
- Diploma in Nursing or a Bachelor Degree in Nursing.
- Registration (active) or ability to register with the Nurses Association of New Brunswick.
- Minimum of two years in the last five years of specialized nursing experience in family practice, surgical, palliative care, or oncology.
- Knowledge of the target population of Extra-Mural Services and available community services.
- Valid New Brunswick Class 5 Driver's License and good driving record.
- Current CPR Level C certification.
- Ability to work efficiently as a member of the team, possess leadership and coaching skills, and demonstrate strong interpersonal skills.
