- You will provide leadership in the development, evaluation, and improvement of clinical practice within a specific clinical area of focus.
- Develop and implement evidence-based clinical care models to achieve organizational goals and objectives.
- Work closely with Home and Community Care Managers to advance clinical practice through program integration and standardization.
