Key Responsibilities- Provide Personalized Care: Deliver nursing care to residents in accordance with established policies, procedures, and recognized nursing principles.
- Assess and Develop Care Plans: Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans to meet their unique requirements.
- Administer Medications and Treatments: Safely administer medications and treatments as prescribed by physicians, ensuring accurate documentation and communication with the healthcare team.
- Maintain a Safe Environment: Identify and mitigate potential safety risks, ensuring a safe and healthy environment for residents, staff, and visitors.
- Collaborate with the Healthcare Team: Work effectively with our interdisciplinary team, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination and communication.
- Document Resident Information: Accurately and thoroughly document resident information, including medical history, treatment plans, and progress notes.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives, identifying areas for improvement and implementing changes to enhance resident care and satisfaction.
- Education and Certification: Current registration as a Registered Practical Nurse with the College of Nurses of Ontario, a strong knowledge of healthcare practices and procedures, and a diploma in nursing.
- Experience: A minimum of two years' experience as a Registered Practical Nurse, preferably in geriatrics or long-term care.
- Skills and Abilities: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and a commitment to providing high-quality patient care.
This position is located in a long-term care facility, and exposure to illnesses and germs is possible on a daily basis. A valid TB test and CPR certification are required. A criminal records check with vulnerable sector screening is also required.
