Job Summary

We are seeking a Clinical Pharmacist to join our Pharmacy Services team at Lions Gate Hospital in North Vancouver, BC. The successful candidate will provide clinical pharmacy services, including individualized patient medication therapy, patient medication counselling, and consultation with physicians and other healthcare providers.

Responsibilities

  1. Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans for patients.
  2. Individualize patient medication therapy by reviewing patient charts, laboratory test results, and medication therapy plans.
  3. Provide patient medication counselling and education on drug therapy.
  4. Assist the healthcare team in developing drug therapy guidelines for individual disease conditions.
  5. Participate in ward rounds, program team meetings, and provision of pharmacy services at the unit level.
  6. Act as a department resource person in the assigned area of responsibility.
  7. Contribute to the development of case studies and review articles for submission to professional journals.
  8. Provide instruction and/or supervision to pharmacy students and residents.
  9. Participate in quality improvement activities and medication use evaluations.
  10. Perform drug distribution duties, including interpreting medication orders/prescriptions and dispensing medications.
  11. Review prescriptions for therapeutic suitability and communicate with physicians and patients as needed.
  12. Ensure that medications are efficiently and accurately dispensed and/or compounded.
  13. Participate in clinical drug trials and medication safety initiatives.
  14. Participate in staff meetings, committees, and other programs as required.
  15. Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics.
  16. Prepare reports on various activities within the discipline.
  17. Perform other related duties as assigned.
Qualifications

Education and Experience

Current registration as a Pharmacist with the College of Pharmacists of British Columbia.

Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.

Skills and Abilities
  • Broad knowledge of organization policies, procedures, and standards of care.
  • Knowledge of evidence-based clinical pharmacy practice.
  • Knowledge of medication use evaluation.
  • Knowledge of research processes and methodology.
  • Knowledge of drug distribution processes in a hospital setting.
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public.
  • Ability to organize and prioritize work.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
