School and Work Transitions Program Counsellor

Hamilton Health Sciences is seeking a skilled School and Work Transitions Program Counsellor to join our team. As a key member of our Oncology outpatient program, you will provide one-to-one vocational and academic counselling and advisement to eligible clients/patients.
Key Responsibilities- Provide vocational and academic counselling and advisement to clients/patients
- Understand and interpret neuropsychological/psychoeducational reports
- Identify education and employment barriers
- Develop client-specific strategies to achieve transition to further education or employment
- Attend secondary and post-secondary schools, places of employment, and community agency meetings as necessary
- Exchange knowledge with healthcare providers, educators, and counsellors
- Participate in educational and informational program presentations
- Actively engage in and promote the POGO School and Work Transitions program initiatives
- Provide information and resources and advocate for survivors and parents
- Liaise with After Care clinic staff
- Establish links with local educational institutions and community agencies
- Participate in the POGO School and Work Transitions Program training initiatives
- Track service delivery volumes and report to POGO
- Provide updates for the POGO School and Work Transitions Program webpage
- Participate in the POGO School and Work Transitions Program evaluation
- Minimum undergraduate degree in Secondary/Post Secondary Education, Psychology, Disability Studies, or related areas
- Minimum 3 years' career and/or vocation counselling experience in a healthcare or education setting
- Experience providing academic advisement to adolescents and young adults
- Experience coaching and supporting others with job preparation, including resume and cover letter writing
- Demonstrated experience providing equitable and inclusive care to parents, adults, and children from historically excluded and marginalized communities
- Demonstrated understanding and commitment to equity, diversity, and inclusion
- Experience with complex case management, outreach, and referrals
- Valid driver's license and access to vehicle
- Knowledge of employment and education barriers for diverse populations
- Knowledge of disclosure and workplace accommodations practices
- Demonstrated client-focused approach to vocational counselling
- Knowledge and demonstrated application of patient-specific community resources
- Familiarity with/knowledge of secondary and post-secondary education systems
- Excellent oral and written communication
- Self-directed and resourceful with demonstrated organizational skills
- Ability to work independently, as well as effectively within a team
- Excellent judgement and decision-making skills
- Strong collaboration and conflict resolution skills
- Experience working with pediatric and adult cancer populations an asset
