Indigenous Patient Care Coordinator

We are seeking a skilled and compassionate Indigenous Patient Navigator to join our team at Interior Health Authority. As a key member of our primary care team, you will play a vital role in providing culturally safe care to Indigenous patients and their families.
About the Role:The Indigenous Patient Navigator will provide consultative and navigational services to health professionals, the community, and clients to understand and identify client needs. This will involve supporting cultural interpretation and/or health literacy translation services, as well as supporting culturally-competent practice. The successful candidate will also provide client support services related to discharge planning, including assisting in the early identification and assessment of clients' needs and liaising with the appropriate community and IH program and services.
Key Responsibilities:- Provide consultative and navigational services to health professionals, the community, and clients to understand and identify client needs.
- Support cultural interpretation and/or health literacy translation services and support culturally-competent practice.
- Provide client support services related to discharge planning, including assisting in the early identification and assessment of clients' needs and liaising with the appropriate community and IH program and services.
- Provide a link between Indigenous communities and IH healthcare providers and support Indigenous peoples' understanding of the provincial healthcare system, the First Nations Health Authority, and federal healthcare systems where applicable.

The Indigenous Patient Navigator will be based in Ashcroft, a small community with a population of approximately 1,800 people. The site provides service to the surrounding area, including the villages of Cache Creek and Clinton, and offers a range of amenities and services, including the Ashcroft Urgent and Primary Care Clinic, Long Term care, Home Care and Dome Support services, public health, mental health services, lab and x-ray services.
The city of Kamloops is approximately an hour's drive away, and Kelowna and Vancouver are approximately 3-4.5 hours away, providing easy access to urban amenities and services.
