Key Responsibilities:

* Work collaboratively with our team to provide individualized support and services
* Develop and implement personalized plans to meet the unique needs of our clients
* Facilitate community inclusion and participation in various settings
* Maintain accurate and detailed documentation of client progress and needs
* Participate in meetings, training, and service planning as required

Requirements:

* Valid Class 5 driver's license
* Related experience working with adults with diverse abilities
* Basic computer skills and physical ability to perform duties
* First Aid certification
* Criminal record check
