Job Title: Community Mental Health Worker

We are seeking a dedicated and compassionate Community Mental Health Worker to join our team at JAZ Community Support Limited. As a Community Mental Health Worker, you will play a vital role in providing support and services to individuals and families in our community.

Job Summary:

The successful candidate will work with clients to assess their needs, develop and implement individualized plans, and provide support and guidance to help them achieve their goals. You will work collaboratively with other healthcare professionals and community agencies to ensure a comprehensive and coordinated approach to care.

Key Responsibilities:
  • Assess client needs and develop individualized plans
  • Provide support and guidance to clients to help them achieve their goals
  • Work collaboratively with other healthcare professionals and community agencies
  • Prepare reports and case histories
  • Perform administrative tasks
  • Implement life skills workshops
  • Assess client's relevant skill strengths and development needs
  • Housekeeping activities
  • Food preparation
  • Provide suicide and crisis intervention
  • Resolve conflict situations
  • Liaise with other social services agencies and health care providers involved with clients
  • Assist clients/guests with special needs
  • Accompany clients to appointments and social outings
Requirements:
  • College, CEGEP or other non-university certificate or diploma from a program of 3 months to less than 1 year
  • Will train
  • or equivalent experience
  • Ability to work independently
  • Work under pressure
  • Physically demanding
  • Attention to detail
  • Ability to work with clients with mental health disabilities
  • Ability to work with clients with physical disabilities
  • Ability to work with clients with intellectual disabilities
