Job Title: Community Mental Health Worker

We are seeking a dedicated and compassionate Community Mental Health Worker to join our team at JAZ Community Support Limited. As a Community Mental Health Worker, you will play a vital role in providing support and services to individuals and families in our community.

Job Summary:

The successful candidate will work with clients to assess their needs, develop personalized plans, and provide support and guidance to help them achieve their goals. You will work in a variety of settings, including community agencies, clients' homes, and various locations. Your primary focus will be on providing support and services to individuals and families, with a focus on mental health, substance abuse, and socio-economic needs.

Key Responsibilities:
  • Assess clients' needs and develop personalized plans to support their goals
  • Provide support and guidance to clients, including life skills workshops and housekeeping activities
  • Prepare reports and case histories to document client progress
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support
  • Accompany clients to appointments and social outings
Requirements:
  • College diploma or equivalent in a related field
  • Experience working with individuals and families in a community setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Valid driver's license and reliable transportation
