Job Summary:

  • To provide basic supportive care as directed by nursing staff, focusing on promoting independence and dignity for residents.
  • To work collaboratively with other healthcare professionals to develop and implement individualized care plans that cater to the unique needs of each resident.
  • To maintain accurate records of resident care, adhering to all relevant legislative acts, such as PHIA and Protection of Persons in Care Act.

Key Responsibilities:

  • Assisting Residents: Providing personal care, mobility assistance, and emotional support to residents, ensuring their comfort and safety at all times.
  • Team Collaboration: Working closely with nurses, therapists, and other healthcare professionals to ensure seamless care delivery and effective communication among team members.
  • Care Planning: Contributing to the development and implementation of individualized care plans that address the unique needs and preferences of each resident.

Requirements:

  • Education: Graduate of an approved Health Care Aide training program.
  • Experience: Minimum 1 year of experience in long-term care or chronic care settings, demonstrating a strong commitment to patient-centered care.
  • Language Skills: Ability to communicate fluently in both English and French (as required by the position).
