Job OverviewThe Pharmacist is responsible for providing clinical services to patients and medical practitioners. This role plays a key part in achieving departmental excellence in patient-focused care, customer service, workflow efficiencies, and PROfile programs.
Key Responsibilities:- Provide clinical services, including disease state management, medication reviews, and health risk assessments.
- Prioritize technical functions for Pharmacy Assistants when available.
- Initiate care plans, follow up, and document patient information.
- Collect relevant patient, drug, and disease information.
- Identify actual and potential drug-related problems and monitor medication adherence.
- Provide patient counseling for new therapy and determine patient understanding of medication use.
To be successful in this role, you will need:
- A strong understanding of pharmacy principles and practices.
- Above-average communication skills (oral and written).
- Strong organizational skills and attention to detail.
- The ability to work independently and as part of a team.
We require:
- A Bachelor of Science in Pharmacy from a recognized institution.
- Licensure as a Pharmacist in the province of responsibility.
- Injection Certification.
