Key Responsibilities- Provide high-quality case management, clinical treatment, and advocacy to clients with Operational Stress Injury, Post Traumatic Stress Disorder, and co-occurring mental health and addiction issues.
- Participate in interdisciplinary treatment team meetings and contribute to resident-centered care planning.
- Coordinate case management and communication across an inter-professional team, including referral agencies, clients, and their families.
- Deliver individual and group therapy sessions using evidence-based modalities, including cognitive behavior therapy, mindfulness, and trauma-informed care.
- Facilitate abstinence-based education and content, and identify treatment goals with patients.
- Collaborate with stakeholders to develop, implement, and follow-up on patient aftercare plans, including referrals to recovery programs.
- Prepare comprehensive discharge summaries and maintain accurate documentation.
- Liaise with patients, families, referral sources, and community providers to ensure seamless care coordination.
- Master's degree in Counselling, Social Work, or Psychotherapy.
- Registration in good standing with a professional body, such as CCPA, BCACC, CCAC, or NSCCT.
- Experience in a residential mental health and addiction treatment setting is an asset.
- Knowledge of abstinence-based community resources and supports is a strong asset.
- Ability to work collaboratively with healthcare teams and communicate effectively with clients and families.
- Exercise initiative and good judgment in a fast-paced environment.
